General Clinical Nutrition Assessment
This tool is designed to support dietitians, nurses, and healthcare professionals in assessing patients’ dietary intake in general clinical settings. It may be used for inpatients or outpatients and, under professional guidance, may also be used by patients themselves.
By entering basic demographic information and recent dietary records, the system generates a nutrition assessment focusing on overall energy intake, macronutrients, and selected micronutrients. The results aim to identify potential nutritional risks and provide supportive information for further clinical evaluation and decision-making.

Dietary Records
To accurately assess the nutritional status of dialysis patients, please record all foods consumed per meal, including breakfast, lunch, dinner, and snacks.
Whenever possible, we recommend weighing each food item. If weighing is not feasible, refer to the standard portion size guide to assist with estimation. The more accurate the entry, the more reliable the analysis.
For each food entry, please provide:

We recommend selecting 1 to 3 representative days from the current dialysis cycle. Up to 12 food entries per meal are supported, which is sufficient for a complete nutritional analysis.
The final report will be based on the average daily intake and include targeted nutrient evaluations and practical dietary recommendations.
